Throughout the medication reconciliation project, I learned a lot about the medications that my client was on and safety concerns that could be a result of taking these medications. While researching my client’s medication, I did notice safety concerns related to a lot of the medication that she is taking. A common side effect that I noticed throughout all of the medications, was fatigue and dizziness. These might not seem like they are bad side effects, but you have to take into account for my client’s age. She is still considered “young” and these symptoms are not common for someone her age. A safety concern that was the most concerning in my opinion was when she is operating a vehicle, for example. If multiple of her medications cause fatigue and dizziness, then driving a car can be scary because she could become dizzy while driving. This is not only a safety concern for only my client, but for other people too. If she was to get in an accident, it could harm other people that would be involved too. My client is able to understand why she is taking her medications and she knows what each medication is for and why she is taking it. It is important that they know why they are taking each of their medications because the client should know if there are any potential interactions with the drug and any adverse effects related to that specific drug. My client is on two medications that do similar things. She is on a T4 and a T3 medication for her thyroid. She’s on these medications because of her history with a thyroidectomy. These medications do not do the same thing, but they are both replacing thyroid hormones that are no longer there, which could be confusing for some people. My client does know why she is on both of these medications and does know the differences between them. While researching my clients medications, I did not see any serious medication interactions that my client had not been aware of. My client is very involved in her own medical care, and knows all that she should about her medications and why she is taking them, as well as adverse effects and any potential interactions that could happen.
